Volunteers under 18 must have a legal guardian read and agree to these terms. Please ensure that the field for legal guardian and birthdate have been completed.

In consideration of this valued opportunity to volunteer, without financial compensation or benefits, to perform services for Orphans of the Storm animal shelter, I agree to the following terms and conditions, intending to be legally bound by them:

1. I will abide by the mission, rules, regulations, policies and programs of Orphans of the Storm. I will follow instructions and guidelines from my supervisor.
2. I assume the risks of being bitten, scratched, injured or frightened by cats, kittens, dogs and puppies while volunteering my services, whether on or off the premises of Orphans of the Storm animal shelter. I understand and accept the risks of working with stray and abandoned companion animals, knowing that (i) they have been or may be, unbeknownst to Orphans of the Storm or me, damaged, sick, feral or abused or (ii) they may attack each other or humans.
3. I am physically capable of handling dogs and cats of all sizes and ages, including those described above.
4. Orphans of the Storm and any other entity, facility or location at which I provide services (including adoptions) on behalf of Orphans of the Storm is not liable to me or my heirs, assigns or representatives for injuries or damages I might suffer or sustain directly or indirectly in connection with the performance of my volunteer activities; nor are any of them liable to me for any losses, judgments, costs or expenses whatsoever I might suffer or sustain in connection with such injuries or damages.
